Below is a list of some of the health insurance plans for which I have agreed to participate.  As large as this list is, it may not be complete, as these things change often.  Your best option in determining whether I participate in your personal health insurance plan is to call your insurance company.   There should be a customer service phone number on the back of your insurance card. Our office does not offer Out of Network rates.
