About Azelvandre Family Practice

The Azelvandre Family Practice was opened in 2006 by Jacqueline Azelvandre D.O. with the intent of providing the same quality health care as her father.  With 34 years of experience, Dr. Jackie continues the family tradition of providing the time and consideration with each patient while keeping the medical costs affordable.

The office is located in the Old City of Longwood at the corner of Milwee St. and Bay Ave. adjacent to the South Seminole Hospital just two blocks north of Hwy 434.

(Tip:  Be careful, some navigational systems place the office on the south side of Hwy 434.  The office is located on the north side of 434, the same side as the South Seminole Hospital.)
